Huntington Senior Care Network Coordinated Care Services

Senior Care Network is committed to the independence of seniors and physically challenged adults. Coordinated care services can help define concerns you have and present possible solutions, by visiting with you in the privacy of your home.

Coordinated cue services include:

  • An in-depth home assessment with client, family members, and support network,
  • development of a written care plan done with the client and family, through referrals and service arrangements,
  • case conferences and coordination with appropriate services,
  • and ongoing monitoring and reassessment to be sure client's needs continue to be met and/or to implement additional services as client's needs change.

Arrangements can be made with community agencies to provide needed services such as home health care, transportation and meals. Huntington Senior Care Network Coordinated Care programs are funded by the Los Angeles Area Agency, Huntington Memorial Hospital and Senior Care Network and provide case management, advocacy, information, and assistance to adult clients who are at-risk of premature and inappropriate institutionalization.
