U.S. PASSPORT CARD

U.S. Passport Card

The new wallet-size U.S. Passport Card is a travel document that can be used to enter the United States from Canada, Mexico, the Caribbean and Bermuda at land border crossings or sea ports-of-entry. Note that it cannot be used for international air travel. The passport card is more convenient and less expensive than a passport book.

The passport card will have the same validity period as a passport book: 10 years for an adult, five for children 15 and younger. For adults who already have a passport book, they may apply for the card as a passport renewal and pay only $30. First-time applicants will pay $55 for adult cards and $40 for children.

The City of La Puente has been designated a Passport Acceptance Facility Service by the U.S. Department of State. Passport applications must be processed in person at the City Clerk Department, 15900 E. Main St., La Puente, CA 91744.

Passport Requirements: Proof of U.S. Citizenship, proof of identity, one passport pictures and appropriate fees. (There are additional requirements for children under the age of 16)

Renewal: The following qualifications must be met in order to qualify for a renewal:

    1. Most recent passport can be submitted.
    2. Applicant was at least 16 years old when the most recent U.S. passport was issued.
    3. The most recent U.S. passport was submitted less than 15 years ago.
    4. The same name will be used on the most recent U.S. passport; or if name has changed by marriage or court order must submit proper documentation to reflect name change.

If travel departure is scheduled within two weeks, it is advised to apply at a Regional Passport Agency. Log onto www.travel.state.gov for more information.

Passport application forms are available in the City Clerk Department during regular City Hall hours, Monday through Thursday, 7:30 a.m. to 5:30 p.m. The execution of applications is limited to passport processing hours. For more information, call the City Clerk Department at 626-855-1522.
